GERIATRIC ASSESSMENT/IN HOME ASSISTANCE

RVNA’s Senior Care Management program to Southbury provides services to help aging residents remain in their homes.  A clinical social worker visits clients in their homes, makes an assessment of needs, and helps residents or family members arrange for services such as a home health aide, housecleaning services, home maintenance, legal assistance, companion services, transportation, and bill paying assistance.

Who is answering the crucial needs of aging in America?

A recent article in USA Today:  “5 must-dos as age wave bears down on the USA” described the five most important issues facing the aging generation.  Based on a survey of 1007 adults 60 and older by the National Council on Aging, United Health Care and the newspaper, it sought data on how well American communities are prepared for the future needs of a growing senior population.

There were mixed results. What was clear were the five areas affecting older adults that directly influence their lives: the need for suitable transportation, more available, affordable healthcare, and senior-friendly housing.  These were the most urgent needs, along with ways to keep seniors engaged and productive and the inevitable need for more caregivers.

Theresa Santoro, President & CEO of RVNA,  [Ridgefield Visiting Nurses Association ] observed: “RVNA’s mission is centered on answering these challenges, for 20 communities in Southwestern Connecticut. We have been providing available, affordable health care from birth to the end of life. For many older adults, our visiting nurses do much more than manage their physical well-being. They listen, make suggestions and provide friendly, continuing concern for their patients, which helps make it possible to avoid nursing homes and emergency rooms.”

Just recently, RVNA became co-sponsor of free, regularly scheduled bus service in Ridgefield.  Eloise Barron, Director of Community Health and Wellness heads an RVNA program: Quality Living at Home. As a Certified Aging-In-Place Specialist, she fills the important but often neglected services and products that make home environments safer and simpler, finding ways to increase mobility for older adults, such as ramps to accommodate otherwise isolated seniors.  Or the installation of bathtub and shower wall bars, plus referrals for hearing and vision problems.  In doing this, RVNA is a pioneer in targeting a national as well as a local challenge.

With RVNA’S oversight, seniors can be more productive, stay more connected and see aging in a new light.  RVNA caregiver training sessions help eliminate the tension and anxiety caregivers often feel.  Caring for caregivers is a major RVNA objective.

Almost half of U.S. adult family members will support a parent or family member at some point.   RVNA has been proactive in caregiver education and respite. The projected increase of the elderly population is staggering. RVNA has been growing along with this population, to create a designation of “Grown-Uppers” who can change the stereotype of “little old ladies” with cheerful, healthy people who are welcome additions to communities near and far. Rehabilitative Therapies in the Home

RVNA offers in home physical, occupational and speech therapy to our community of valued residents.  Following a fall or operation, it is normal to feel somewhat vulnerable and insecure.  Those feelings compound as you age.   Most seniors report that they prefer to stay safe at home during the rehabilitation process.  To address this valid need, RVNA has developed an experienced team of rehabilitation therapists equipped to travel to homes throughout Western Connecticut including Southbury, Woodbury, Danbury, Bethel, Newtown, New Milford, New Fairfield, Brookfield, Redding, Weston, Easton, Oxford, Wilton, New Canann, Georgetown and Ridgefield.  Our team specializes in providing these essential therapies where the patient wants it most – in the comfort and security of their own homes.

As Rehabilitation Manager, Gigi Weiss, PT and her team of 13 professionals, provides physical, occupational and speech therapy.  Occupational therapists help patients help themselves by providing techniques and therapy so they can perform their routine daily activities.  Speech therapists help with language and swallowing issues.  Other services include Kinesio Taping, a rehabilitative taping technique that facilitates natural healing processes and Lymphedema therapy to help reduce swelling and control pain.  RVNA offers a balance and vestibular rehabilitation program, pre and post joint replacement program and a falls prevention and home safety program.  If you qualify medical insurance will cover the cost.

A registered nurse is available 24-hours a day for at-home care in Southbury area. Nursing care is also available by appointment through visits to the VNA office.
